Upon checking in at our front desk and you will be given patient registration forms to fill out that includes your insurance information, general medical history and patient consent. It is important to complete the medical history page accurately as it will be reviewed by the physician during the office visit. Once completed, it will be submitted back to the front desk staff along with your license and insurance card. Your information will be used to create your electronic file in our software.
A biometric screening helps to determine your risk level for certain diseases and medical conditions and consists of a blood draw and other basic biometrics such as blood pressure, height and weight, and BMI index.
Physician Interview and Assessment
Our team of board certified and licensed physicians will take the time to sit with you during your physical to review your medical history, biometric screening results, lifestyle, and any concerns that you have, and will conduct physical examinations of your heart, lungs, head and neck, abdomen, etc. All of these factors will allow the physician to determine what you may be at risk for and which ultrasound exams to order.
Ultrasound imaging is a non-invasive medical test that produces pictures of internal body structures that helps physicians diagnose and treat medical conditions. It is a safe and painless technique that involves the use of a small transducer (probe) and ultrasound gel placed directly on the skin. High-frequency sound waves are transmitted from the probe through the gel into the body. The transducer collects the sounds that bounce back and a computer then uses those sound waves to create an image.
A Doppler ultrasound study may be part of an ultrasound examination. It is a special ultrasound technique that evaluates blood flow through a blood vessel, including the body’s major arteries and veins in the abdomen, legs, neck, and head.

Once you have completed the physical you will go back to the front desk to check out. A letter will be provided to you letting you know when to expect your results and what will happen if follow up treatment is required. In the case where the physician recommends a follow up nuclear stress exam, the front desk can schedule your appointment at our medical facilities on the spot and provide you with preparation instructions for the exam.
The results of your physical will be available within 5 business days from the date of your physical and it will be accessible via your online wellness portal. You will receive a welcome email after your exam and once you complete the registration process and health risk assessment, you will be able to view all of your results.
